About Us

The Lake County Continuum of Care (LCCoC) is a coordinating group that aligns resources to facilitate solutions to end homelessness in our community of Lake County, California.

A Continuum of Care (CoC) is a regional or local planning body consisting of service providers, lead agencies and individuals that work together to assist adults, youth and families experiencing homelessness and to provide the services needed to help such individuals move into transitional and permanent housing, with the goal of long-term stability. This HUD program is designed to:

  • promote community-wide planning and strategic use of resources to address homelessness;

  • improve coordination and integration with mainstream resources and other programs targeted to people experiencing homelessness;

  • improve data collection and performance measurement;

  • and allow each community to tailor its program to the particular strengths and challenges within that community.

 

A lead agency or Administrative Entity (AE) is designated to carry out the day-to-day activities of the CoC and its grants including fiscal and compliance activities. Lake County Behavioral Health Services is the current AE for the Lake County Continuum of Care.

In addition to an AE, the Coalition has Agencies that provide leadership for key CoC activities. Those agencies include:

  • Adventist Health Clear Lake - Point-in-Time Count Committee Chair

  • Hope is Rising - Secretary of CoC

  • Lake County Behavioral Health Services - Administrative Entity and HMIS Administrator

  • Lake County Office of Education - Strategic Planning Committee Chair

  • Praises of Zion - Performance Review Committee Chair

  • Lake COunty Board of Supervisors - CoC Vice Chair

  • Employment Development Department - CoC Chair

The LCCoC is governed by a volunteer board of directors and its coalition members. The LCCoC hosts public monthly meetings and all are invited to participate, although voting is restricted to CoC members.
